Applying for a Replacement Charitable Collection License

The Chief Executive may issue a replacement licence to a licensee if satisfied that the licence has been lost, stolen or destroyed. To assist licensees apply for a replacement licence, it is necessary for you to complete an 'Application for a Replacement Charitable Collection Licence' form in accordance with section 28 of the Act.
